Please log onto MyDegreePath to view your Degree audit. If you have any questions or see any discrepancies on your degree audit, complete this webform and attach supporting documentation.
The following are common discrepancies or missing items to look out for when reviewing your degree audit:
- Incomplete course requirement - (if applicable) provide documenation of:
- approved General Petition or course waiver that has not been applied to your degree audit
- approved Course Reduction form (ES and ME graduate programs only)
- unoffical transcript showing proof of completed letter graded course
- Incomplete technical seminar requirement - provide documentation of your talk or conference acceptance (date/conference name).
Please indicate the reason for the submission below. Allow 3-5 business days for us to review and respond to your submission.
